Order ticket  

Definition 1.

A form detailing an order instruction that a customer gives an account executive.
 

Definition 2.

Form completed by a Registered Representative of a brokerage, upon receiving order instructions from a client, with all the relevant information about the order, including type of order, security name, number of shares, and any special instructions. also called ticket.
 

Definition 3.

A form completed by a registered representative of a brokerage firm upon receiving order instructions from a customer.
